Moreover the need of all doctors, nurses to regularly write SOAP notes (Subjective, Objective, Assessment, Plan) after each examination and round is desired as archives and records management share a simple goal of providing for organizational & individual accountability. Accountability depends on being able to demonstrate managed access to information which is important for reasons of ongoing need or future evidence, from the time of its creation.

Document everything you do; as it is your documentation which is only going to be of any use later, he added. He conveyed that at OCHRI nobody is expected to follow oral instructions. Nagpur Today : Nagpur News

The Maharashtra’s Organ Transplant Act will now be called Organ and Tissues Act.

Nagpur: In a major decision, the Maharashtra Government has accepted the amendments made in the Central Government’s Human Organ Transplant Act, 2011, announced the Medical Education Minister Vinod Tawde in Assembly on Day-5 of Winter Session. Tawde further said that Central Government had passed the Human Organ Transplant Act in 1994. The State Government too started to implement and follow this 1994 Act toto. However, after experiencing certain shortcomings, the Central Government included some amendments and passed the amended Act in 2011, informed Tawde. Later, Tawde moved the amended Act in toto with a resolution to include some provisions in the Act in future. The resolution was passed by majority in the House.

The State’s Organ Transplant Act will now be called Organ and Tissues Act. Among the changes in the new Organ and Tissue Act, paternal and maternal grandparents and grandchildren have been included in near relatives,” said Tawde. The Act now allows swap transplant in the state. “Although swap transplants were being conducted in the State, it was done because the Central Act allowed it for the benefit of the patient. Necessary permissions were taken for the same. It will now be done officially,” said Tawde.

An unrelated donor could donate his or her kidney to a recipient whose family member would in return donate one kidney to another patient, according to the Central Act. “This method is successful for those whose blood group does not match within the family and require kidney donation from a stranger,” said the Medical Education Minister.

Nagpur Today : Nagpur News

Nagpur: The Government Medical College and Hospital (GMCH) administration on Monday initiated the process to start the much awaited trauma centre. It expects to put the basic equipment, medical and paramedical staff in place by the month-end and get the centre inaugurated by the last week of January.

GMCH dean Dr A Niswade held a meeting of department heads, PWD engineers, nursing heads and other authorities to set the ball rolling for the formal launch of the trauma centre. Basic equipment like CAT scan, C-Arm (equipment required for orthopaedic surgery), digital subtraction angiography (DSA) etc will be installed in about a fortnight. The gas pipeline for oxygen will also be installed before the trauma centre gets going.

The dean has already identified security personnel, nursing staff and teachers from orthopaedics, surgery and neurology departments to be posted in the centre. “The staff will be appointed on state government instruction on contract basis,” said Niswade. But security staff needs to be put in place first, to safe guard the expensive equipment during the installation process.

There is a big hall at the entrance, where the casualty staff will identify patient for degree of seriousness or intervention required. A red band will be tied to most serious patients, a yellow one for medium and green one for less serious patients, for better handling and management of patients by attending staff, who would refer them to expert doctors.

The dean plans to start the trauma centre initially with 30 beds instead of the purposed 90 beds for the centre. These will include 10 beds in the casualty, 10 ICU beds and 10 beds in ward.

Medical education minister Vinod Tawde had in 2014 winter legislative assembly session promised to make the centre functional within three months. But the centre couldn’t take off even one year later. Now, even when the dean is planning to start the centre, it is not at its full capacity. The centre is being set up with financial support from both central as well as state governments.

Nagpur Today : Nagpur News

dds_1452640761Nagpur: Miracles do occur, they say. The Almighty often reminds us that He is very much present and does miracles every now and then. Once such miracle was observed when a woman delivered quadruplets in Lata Mangeshkar Hospital in Digdoh, Hingna.

This proud mother of the quadruplets is identified as Kalpana Chauhan. The ‘miracle’ occurred at 11:15 am on January 12, 2016. Medical professionals claim that this occurs when four healthy spermatozoons enter four healthy and fertile eggs or ovules.

This 24-year-old woman from a small village of Hingna delivered four baby girls in city’s Lata Mangeshkar Hospital (LMH). Doctors say that bearing quadruplets naturally is so rare it happens to one in more than four crore women, the probability of this being 1:80^4. Being born only 26 weeks (normal gestation period is between 32 to 34 weeks), all four babies are way below the normal birth weight and have been admitted to the neonatal intensive care unit (NICU).

Kalpana Chauhan was told she was carrying three babies after the initial sonography. Considering it a high-risk case, doctors at LMH had admitted her to the hospital during her sixth month of pregnancy to monitor her continuously. She left the hospital against medical advice a week later.

On Monday morning i.e. January 12, 2016, she had visiting the hospital for a follow-up when doctors realized she was already in advanced labour. Within forty minutes of being in the labour room, she delivered the four babies, the fourth one being a surprise for both doctors and the parents.

While speaking to Nagpur Today, Head of the Gynaecology Department of Lata Mangeshkar Hospital Dr Sulbha Joshi said that multiple pregnancies occur commonly among women being treated for infertility. However, bearing four children at the same time is very rare. This is the first case that she has seen in thirty years where quadruplets were borne spontaneously. She added that the delivery was normal and was done by Dr Yamini Kale from her Department.

Head of the Paediatric Department of Lata Mangeshkar Hospital Dr Nilofer Mujawar said that at birth, the babies were all below 700gms (the normal birth weight is above 2kgs). She claimed that they have admitted them to the NICU. The babies are still in a critical condition. The NICU team headed by Dr Meenakshi  Girish, is giving them the best possible care.

Dean Dr Kajal Mitra informed that considering the financial condition of the family and the nature of the case, LMH has decided to provide the NICU services and investigations free of cost.

Nagpur Today : Nagpur News

nagpur live 2016Nagpur: The need for performing certain cardiac procedures like bypass surgery and angioplasty is a much debated topic among doctors as well as patients. The good news is that some of the new-age drugs negate the need for such procedures. The next generation of these medicines are being developed with many more advantages that the cardiologists are excited about.

This was among the discussions during ‘Nagpur Live 2016′, a cardiology conference being organized jointly by Health Education and Research Trust, Nagpur, Arneja Heart And Multi Speciality Hospital in collaboration with Vidarbha Chapters of Cardiological Society of India (CSI) and Association of Physicians of India (API). Faculties of the conference include Dr Ramakant Panda from Mumbai’s Asian Heart Institute, Dr AB Mehta from Mumbai’s Jaslok Hospital, Dr Ashok Seth of New Delhi’s Fortis Escorts Heart Institute and Dr Tan Huay Cheem from Singapore.

Explaining the physiology of heart attacks, Dr Mehta said, “Cholestrol deposits known as plaques cause blood vessels to narrow down. Being covered by thin capsules and rich in lipids, they are liable to rupture. This can cause a clot that completely blocks the vessel. Such blockages are often the first indication of heart trouble for several patients.” Regular screening of people vulnerable to heart ailments and treating them aggressively with stents, especially among people who are vulnerable having been exposed to more than two risk factors can help avoid unnecessary surgeries, he said.

“There is, in fact, an older technology called fractional flow reserve (FFR) that helps arrive at a numeric value of the internal pressure in the blood vessels. This measurement of the pressure can help us decide whether there is a need for a surgery or medicines will be enough to treat a patient,” informed city-based cardiologist Dr Jaspal Arneja. He added that 80% of the blockages in cardiac vessels can be managed without surgeries, with 15% requiring bypass and only 5% requiring angioplasty.

Dr Cheem who is the president of Asia-Pacific Society of Interventional Cardiology was very upbeat about the next generation of cardiac medicines. “We are currently using first generation medicated stents, or mesh tubes, that can be absorbed in the bloodstream in a couple of years’ time. There are several contra-indications or conditions under which these stents should not be used. The second generation of stents that are being developed will be available for use in a year or so. These will help overcome some of the prevalent issues of using these meshes,” he said.

The improvements being talked about include thinner covers or struts that is likely to reduce the time of absorption of the stent, added Dr Cheem. “The stents become vestigious after a couple of months of being put in the body. Therefore, their absorption at a time earlier than a couple of years is desirable considering they are made of metals. Also, the number of contra-indications of these new-age stents will be lesser,” he said.

Modelled on India live, the biggest academic fest for cardiologists in the country, the first Nagpur live conference was attended by more than 700 doctors from across the country. There were 45 faculties who will deliver 100 lectures over the two day event.

The uniqueness of the event was live surgeries performed at Arneja Hospital being beemed to the delegates. The guest faculties and delegates could interact with those performing the surgeries, asking them reasons for making uses of a particular technique.

Faculties, including the most distinguished cardiologists, were impressed with the standard of discussions and the technology available in the city. They felt it was comparable with that in the metros.
